provide any treatment which is considered to be

- Know the name and role of the health care providers involved in your care

- Identify a spokesperson with whom we can communicate or provide a valid power of attorney for personal care in the event that you cannot make decisions for yourself

- Participate in all decisions regarding diagnosis, treatment options and discharge planning

- Inform your health care providers if you do not
understand or cannot follow the health care instructions

- Have your questions and concerns answered in terms that you can understand

- Express any concerns regarding your care to the health care providers or to the patient representative

- Make informed choices about your care and available options
|
- Take care of your personal property and valuables and send home anything that is not immediately necessary

- Encounter coordinated care from skilled
health care providers

- Treat staff members and other families with respect
|
- Be informed that TEGH is committed to participating in medical/clinical teaching - clinical students may be helping in your care, under supervision
|
- Recognize that the needs of other patients and families may sometimes be more urgent than yours
|
- Be informed of all expenses not covered by OHIP
|
- Respect hospital policies and property
