CLINICS - OUTPATIENT (AMBULATORY CARE SERVICES)

Ambulatory Care Services provide timely access to physician specialists and to programs targeted at prevention, assessment, diagnosis, treatment and ongoing care. Our goal is to increase healthcare efficiency by providing contemporary health programming and services that address the changing health care needs of the South East Toronto community and beyond.

Hours of Operation:
Monday through Fridays from 8:00 am to 4:00 pm

Booking an Appointment
Call (416) 469-6031 or Fax a referral form (416) 469-6507 to book an appointment in one of our clinics. The following information is required

  • name
  • address and phone number(s)
  • date of birth
  • reason for visit (diagnosis)
  • name and address of referring physician
  • results of relevant tests

A referral from a Family Physician is required except for the Chiropody service.

What to Expect:
Every effort will be made to see patients at their appointment time.  However, as emergencies can arise and patient volume is unpredictable, please be advised that you may experience extended wait times.  To make your visit as pleasant as possible please bring the following to your appointment:

  • Valid Health Card issued by the Ministry of Health and Long-Term Care.
  • List of medications that you are currently taking.
  • If you have had x-rays done at a location outside the hospital please bring them to your appointment.
  • Money for food or drink
  • Change for the parking meter
