FAMILY HEALTH CENTRE

Established in 2002, over ten family doctors, several first and second year University of Toronto residents and medical students in the Family Health Centre serve the medical needs of the South East Toronto community.
The Family Health Centre provides exceptional patient based care.

Services:

Low-Risk Obstetrics
A number of physicians within the Family Health Centre participate in the Low-Risk Obstetrics Group.
Responsibility of taking care of the patients is shared among the care providers and is available 24 hours a day, 365 days a year.

Dietitian and Psychotherapist
The Family Health Centre provides patients with a dietitian and psychotherapist on staff.
For further information please speak to your Family Health Centre physician. 

Residency Practice
The Residency Practice provides resident-physicians (a physician who is a medical school graduate but is training to specialize in Family Medicine) with an opportunity to interact and service the medical needs of the South-East Toronto community.
The benefit is two-fold.  Eager and qualified resident-physicians receive hands on learning and training and patients receive exceptional and attentive care.
Resident-physicians participate in the Residency Practice for two years.  Once the resident-physician completes his/her training a new resident-physician will provide medical care to the patient.
The Family Health Centre continually strives to improve patient care.

Family Health Group
Physicians at the Family Health Centre became part of the 840 Coxwell Family Health Group in 2004.  This change greatly enhanced the availability, accessibility, and quality of care for patients within the Family Health Centre practice.
Patients have access to Telephone Health Advisory Service where a nurse is available 24 hours a day, seven days a week.

Electronic Medical Record (EMR)
The Family Health Centre is one of the first family medicine units in east end Toronto to become an EMR facility.
EMR is an electronically generated medical chart.
The use of EMR has improved patient care efficiency and accuracy.

Hours of Operation
:

Monday to Thursday - 8:00 am to 8:00 pm
Friday - 8:00 am - 5:00 pm
Saturday - 9:00 am - 12:00 pm (Emergency walk-in service for patients of the Family Health Centre only)
