The Complex Continuing Care & Rehabilitation Health Service provides non-acute care services to an adult population who meet specific admission criteria. Our service is committed to providing high quality, compassionate care, which respects and values each individual and focuses on informed health management, individual choice and quality of life.
There are two components of the Health Service:
- Complex Continuing Care
- 75 beds on J5 and G5.
- In patient Rehabilitation
- 13 Short Term Inpatient Rehabilitation beds and 8 Alternate Level of Care (ALC) beds, located on A2.
Complex Continuing Care Service

General Complex Continuing Care: This program is designed for individuals with medically complex care needs who require the services of our multidisciplinary care team. These residents are often totally dependent on health professionals for all their care needs. We provide care that is resident focused and has an emphasis on compassion, caring and improving quality of life. The length of stay can be months to years.
Medium Intensity Rehabilitation: This program is designed for individuals who are unable to tolerate intensive rehabilitation programs. Working with our multidisciplinary care team, we anticipate that patients will show functional improvement and meet therapeutic goals within six weeks to three months.
Respite Care: We offer three respite care beds. Admission to these beds is pre-arranged and must have a definite discharge date. The length of stay for the respite patient can be anywhere from a few days to a maximum of three weeks at any one time. Palliative Care: This program provides supportive and compassionate care and focuses on quality of life for those living with a disease that is not responsive to curative treatment. The program is designed for people in the terminal stage of illness with a prognosis of less than three months.
Chronic Ventilator Assisted Care: This program provides a compassionate and stimulating environment that enhances the quality of life for our ventilator assisted residents. The individuals admitted to this program require mechanical ventilation for all or part of a 24-hour period.
Rehabilitation Services
Short Term Inpatient Rehabilitation – A2 This program provides shorter-term, multidisciplinary inpatient rehabilitation to medically stable individuals who are returning to the community following an acute care stay at the Toronto East General Hospital. There is a focus on health promotion through interventions aimed at both recovery from and prevention of impairments and disabilities.
Alternate Level of Care This program provides multidisciplinary care in partnership with the Medicine Health Service to individuals awaiting long-term placement at another facility.
